1. Fill-up the ONLINE ADMISSION FORM.

    2. An email will be sent to you confirming your application.
    3. Scan or take photo in clear copy and e-mail to: ccdisorsogonshs@gmail.com for Senior High School and ccdisorsogonadmission@gmail.com the following admission credentials:
      Senior High School

      • 2×2 picture
      • Junior HS  Report Card
      • Good Moral Character
      • NSO Birth Certificate

      College (Freshmen)

      • 2×2 Picture
      • Report Cart (F138)
      • Good Moral Character
      • NSO Birth Certificate

      College (Transferee)

      • 2×2 Picture
      • Honorable Dismissal or Certificate to Transfer
      • Honorable Dismissal or Certificate to Transfer
      • Certificate of Grades of Transcript of Record
      • NSO Birth Certificate

    4. You will be e-mailed with the Matriculation Form with subjects, assessment of fees and schedule to pay the enrolment fee at CCDI accounting office.
    5. Pay the corresponding enrolment fee and submit the original copy of the above-mentioned requirements.
    6. Once paid, you are given with an Official Account to CCDI Learning Management System.
    7. Congratulations! You are now officially enrolled.
